This is not an accident investigation report.

On 8th May 1978 flight 193 a Boeing 727-235 crashed at 9:20 pm. while attempting to land, coming to rest in 12 feet of water. It was reported that the their was a failure to monitor altitude and descent rate by the crew. There were 58 aboard the jet, there were three fatal injuries.
